As we embark on our third year, we continue to refine our goals and also seek further representation from members in our region and on our Board of Directors. Please note funding is available to people who participate.
Goals for Year 3
Expand FPN Membership: Grow the membership of family practitioners who practice in the Mid-West Region.
Recruit for Leadership and Working Groups: Recruit new members for the BOD and working groups.
Advance Digital Technology Integration: Support the spread of digital technology (on-line booking/e-referral/eConsults, and integration of AI tools).
Enhance Access to Mental Health and Resources: Disseminate information to improve access to mental health and other resources in the Mid-West Toronto region.
Strengthen Primary Care Specialist Collaboration: Continue the primary care/specialist collaborative to address improving coordination and patient co-management.
Guide and Support Regional Initiatives: Inform priorities and contribute to the planning and implementation of Mid-West Toronto Region and OHT initiatives.
Improve Patient Attachment to Family Physicians: Enhance efforts to both strengthen existing relationships and attach more patients to establish lasting connections with family physicians.
Build the Mid-West Toronto Family Practice Network Brand: Develop and promote a strong, recognizable brand for the Mid-West Toronto Family Practice Network.
Refine Governance Policies: Continuously enhance and fine-tune the governance policies of the Family Practice Network for effective and transparent management.
Letters of Support
The Family Practice Network offers letters of support to partner organizations and clinics applying for funding applications. These letters are provided to those who meet specific criteria, as determined by the Board of Directors. Annual General Meeting Summary
We were pleased to host our second Annual General Meeting at the Faculty Club, University of Toronto, on June 7, 2023. A total of forty-eight members atended in person or virtually: the majority partaking of wine and canapés on the patio during the first hour of the meeting.
Our business meeting began with our co-chairs, Michelle Naimer and Pauline Pariser, giving the year in review, emphasizing the strides we have made in onboarding members to On-Line Appointment Booking and OCEAN e-referral, growth of programs supporting people with mental health challenges and homebound older adults and the launch of our primary care/specialist collaborative aiming to improve communication and coordination between our two sectors.Recent opportunities to enhance management of Congestive Heart Failure and Limb Preservation for people at risk for lower limb amputation were presented, as well as a description of the efforts that were made to inform the recent Expression of Interest to fund a range of positions that will build team-based care in Mid-West Toronto.
