Led by the Mid-West Toronto Ontario Health Team and Access Alliance, Open Door breaks down healthcare barriers by providing services directly in the community through:

What does Open Door offer?
We provide a range of health-related community outreach services, events, and communications, including:
Screening and assessment: Identifying immediate and current health needs, and determining the need for further evaluation, treatment, and support in areas such as blood pressure, mental health, lower limb preservation, and lung health.
Information and system navigation: Providing information regarding a wide range of health and community services and resources and warm referrals as needed.
Health Education: Educating communities about health and wellness to encourage healthy behaviors and prevent or manage chronic illnesses.
Coaching: Assisting individuals in identifying and achieving health goals to improve overall well-being.

Who does Open Door serve?
Open Door targets communities and groups facing structural barriers that limit their access to healthcare, including people with disabilities, low-income residents, newcomers, seniors, and those with limited language skills. We operate directly within these communities to bridge the gap between health and social services.

Open Door connects residents to resources, simplifies the healthcare experience, and empowers community members with preventative care guidance. We focus on health goal-setting and building trusting relationships to create a healthier, more connected community.

The Open Door program has made significant strides in connecting underserved communities to essential healthcare and services.
Key Impact Highlights:
660+ individuals have been attached to primary care, ensuring ongoing health support.
Over 1,500 referrals have been made for vital screenings, including pap tests, mammograms, and colorectal screenings, helping prevent and manage serious health conditions.
Funding secured through 2027, ensuring the program’s continued success and reach.
