About the Family Practice Network
The Mid-West Toronto Family Practice Network (MWT-FPN) is a non-profit corporation that represents all interested family practitioners who practice in the Mid-West Toronto region. The MWT-FPN works to improve, coordinate, and act as the leading voice in primary care in the healthcare system and acts as a key partner to our MWT-OHT.
The information below is primarily written to inform primary care providers of the benefits of the Network.
Currently, there are 315 members and the MWT-FPN is open to any family practitioner in the Mid-West Toronto region to join. Membership is voluntary and not legally binding as members can withdraw at any time. Members are kept informed of, and have the opportunity to influence change affecting primary care in our region– whether it’s about new services, accessing resources, or plans or policies that are being contemplated by the Mid-West Toronto Ontario Health Team that has an impact on family health practitioners.

Our Mission
We aim to become the representative voice of family practice in our region to promote collaborative and coordinated care between the community and hospital sectors for our diverse patient populations.
Our Values
We believe that excellent health care needs coordinated local family practice leadership and engagement. We strive to create a network that is:

Board of Directors
The MWT-FPN has a Board of Directors that inclusively represents all practice models; Family Health Teams (FHTs), Family Health Organizations (FHOs) Community Health Centres (CHCs), small practices, Family Health Groups (FHGs), Comprehensive Care Models (CCMs), Fee for service solo physicians, and hospitals. There is a transparent process to select board members outlined in our governance documents based on member consensus.
