Helping Individuals Transition from Hospital to Home with the Support They Need
The Next Step program, formerly known as CAMH@Home, provides a critical transition for older adults living with mental health challenges, including schizophrenia, schizo-affective disorder, and bipolar disorder, who are moving from hospital-based care to community-based services.
This innovative Mid-West Toronto Ontario Health Team program helps people continue their recovery in the comfort of their own homes, ensuring they receive the right level of care and support in the community.
How It Works
The Next Step program connects individuals who no longer require intensive hospital care, but still need moderate support to prevent readmissions. By collaborating with community service partners, we ensure clients are matched with appropriate services tailored to their needs. The program includes:
Personalized Care Plans: An interdisciplinary team works with each client to create a plan based on their unique goals and requirements.
Ongoing Support: Clients receive continuous care, helping them stay well and integrated in the community.
Community Partnerships: The program works with trusted local providers such as Reconnect Health Services.
Available to Those in Downtown Toronto

How to Access The Next Step
The Next Step program is available to CAMH patients who are transitioning from the outpatient Flexible Assertive Community Treatment (FACT) teams to community-based services. If you or someone you know is a CAMH patient who no longer requires intensive care but still needs support, this program can help.
To access the program:
CAMH healthcare providers can refer individuals to The Next Step program during their transition from the FACT team.
