New: 2023/2024 Homebound Influenza and COVID-19 Vaccination Program run by Toronto Paramedic Services- Accepting Referrals

Toronto Paramedic Services’ (TPS) will be offering influenza and COVID-19 vaccinations to homebound patients. Referrals to TPS should only be made by PCPs when the provider is not able to administer vaccine to the homebound patient.

Primary care providers:

  • Identify patients who qualify.

  • Get consent for homebound vaccination.

  • Refer eligible individuals using the updated template.


Homebound Patient Definition:

  • Individuals with medical, cognitive, or psychosocial needs/conditions that inhibit them from visiting a pharmacy or a primary care provider to receive influenza or COVID-19 vaccinations.

  • Transportation and other strategies to remedy access issues have been considered but are not available or not appropriate in the individual’s circumstances.

  • Live in the City of Toronto (have an “M” postal code address).


NOTE: Please ensure that the referred patient is aware that they will be screened again during the booking process with TPS. If TPS determines the patient is not homebound, the patient will be referred to a community vaccination site (pharmacy or city clinic).

NEW -Virtual Urgent Care Clinic at CAMH

CAMH has launched Canada’s first and only same-day hospital-based Virtual Urgent Care Clinic, dedicated to providing urgent support for people in need of urgent mental health care, including addiction, advice. The virtual clinic is open to patients over age 16 years who live in the City of Toronto and are concerned about their mental health and able to wait to speak with a mental health provider later the same day.

Patients book an appointment themselves directly through an online portal at camh.ca/virtualurgentcare from 9-5 pm daily. Evening and weekend appointments will be available.  Patients will need to be able to speak in a private space using a computer, smartphone or tablet. See the attached flyer which you can share with your patients so they can book their appointment.

One Stop Talk/ Parlons Maintenant

One Stop Talk/Parlons maintenant (OST/PM) offers free virtual single session counselling services and seamless entry into child and youth mental health services across Ontario. The program serves children and youth aged 0-17, connecting them with registered and experienced therapists across the province as a single access point. Parents can attend sessions with their child(ren).

When is OST/PM available?

  • Monday to Friday: 12 PM to 8 PM

  • Saturday: 12 PM to 4 PM

 

Service Details:

  • Focuses on addressing one issue at a time and provides the freedom to access the service as many times as needed.
    OST/PM is not anonymous – the program requires children and youth to provide information about themselves, including their name, date of birth, and postal code, to receive service.

  • Start with a conversation, leave with a plan. No matter why you connect, children and youth co-create a personalized plan with their therapist, which is shared with them.

  • Navigation services are available if additional assistance beyond the session is needed. OST/PM directly refers them to additional services as close to them as possible in the child and youth mental health sector.

  • OST/PM has dedicated Francophone clinicians and interpretation services to over 200+ languages, including ASL, ensuring children and youth can receive high-quality mental health support in their preferred language.

ECHO Programs

Are you interested in interactive case-based learning opportunities?  ECHO programs support healthcare providers through interactive case-based learning on complex chronic conditions. All primary care providers from your team, and across the province, are invited to register. 

What is ECHO? 

  • ECHO links primary care providers in Ontario with each other and with a specialist team during weekly videoconference sessions. 

  • Participants gain knowledge, skills and support. 

  • Each session includes a short didactic and real (de-identified) case discussions.

  • Participants earn CPD hours, and there is no cost to join (MOH funded).

NEW!  Women’s Health Procedure Clinic at the Mount Sinai Academic Family Health Team 

Created in order to provide timely access for IUD insertions/removals, contraceptive implant insertion/removals (Nexplanon) and endometrial biopsies. Please find attached the clinic referral form that we ask for you to complete in its entirety and fax back to the number on the form. 

For those of you in a FHO model please note that completion of this referral form allows the Women’s Health Clinic physician to use a billing code that will not negatively impact you're outside use.

When referring your patients:

  1. Please remind your patients that this clinic is being held at the Mount Sinai Academic Family Health Team  located at 60 Murray Street, 4th floor Family Medicine Centre– NOT in the main Mount Sinai Hospital building.

  2. Please provide your patient with a prescription for Nexplanon or the IUD.

  3. Please advise the patient to bring the IUD/implant to their Women’s Health Clinic appointment.

  4. Please ask the patient to take 400mg of ibuprofen 1-2 hours before their scheduled appointment time (if no contraindications).

  5. Note that any abnormal laboratory results will be communicated directly to the referring physician to ensure that an appropriate follow up plan is in place.
